it's not about who should , it's about feeling , you can say I love you to anybody without any feelings too...

We don't need also say " I love you " at all.

simple : did you eat ? Are you okay ? Have you sleep good etc .... this words showing that we really care about person and sometimes this words can replace
" I love you " .
